Services by phone, mail or email to get answers to questions about:
- Direct stock purchase plan, known as the Shareowner Service Plus Plan. Click here for a prospectus,
forms and online enrollment.
- Dividend reinvestment.
- Direct deposit of dividends.
- Dividend replacement checks.
- Photocopies of cashed dividend checks.
- 1099 tax forms, transfer instruction.
- Replacement of lost certificates.
- Account information.
Shareholders will receive an acknowledgment of their inquiry by email within one business day of receipt
of the electronic message and, in most cases, the request will be completed within 48 hours.
Representatives of EQ Shareowner Services are available from 7 a.m. to 7 p.m. Central Time Monday
through Friday to answer your questions. A company Shareholder Services representative is also available
to assist you.
